Health Forms and Safety

Safety is our first priority, no matter what activity children are engaged in or where we are. In addition to having a certified Health Director on site, all of our Head and Associate Counselors are CPR and First Aid certified. We bring water, first aid supplies, an emergency contact list, cell phones and walkie talkies to all parks and field trips away from the LREI campus.

We strive to meet the needs of all children. If your child takes medication, is physically challenged, or has special health needs, please let us know when submitting registration and medical forms so that we can make plans to accommodate them in the most appropriate way possible.

Camp Health Form

Camp Medication Form (Optional): If you would like for our Health Director to be able to administer over the counter or prescribed medication to your child during the camp day, please fill out and sign this form and email to [email protected] by June 1st*Please note that prescription medication requires a signature from your Health Care Provider.

Allergy Action Plan: If  your child has allergies that could require medication (EpiPen, or Benadryl, etc) please be sure to fill out the Allergy Action Plan form and email to [email protected] by June 1st.

If your child requires a SEIT this summer, download the SEIT Form and email it to [email protected] no later than May 15th, 2024.  We cannot accommodate SEITs without prior notice and receipt of form.  Please note that we only allow one SEIT per Homegroup and we reserve the right to place your child in a different Homegroup based on availability.
